Reach the hidden cave beyond Kallias’s crypt
Continue Lacra’s questline through Our Rotten Roots, wait for the next meeting and speak to her near Westridge Outpost. Follow the wisps through the swamp and enter the sunken crypt during The Night of Horrors.
After inspecting the sarcophagus and defeating the spirits, finish the conversation and inspect the blocked wall. Lacra opens a way into the hidden cave. Enter the next chamber and speak with her beside the mandrake.
Eating the root and drinking Lacra’s blood are separate
Accept Lacra’s offer to eat the mandrake to unlock Mandrake Ward. Its death-prevention effect is limited to once per night; it is a persistent perk obtained from this choice, rather than a repeatable healing consumable.
The conversation then moves to a possible romance scene. Following that route and drinking Lacra’s blood unlocks Mesmerise. You do not need that later choice to identify which reward comes from the mandrake itself.
